Fixes font issues in LPT files (see #3005). LPS files are still an issue. The core problem involves font data (size, underline, bold, italics, etc) that are stored in the file as an integer, but represented in the common data architecture as a mix of int/string.

Updated manipulator code to keep CFvw3DManipulator as similar to osgEarth's EarthManipulator? as possible while keeping the "thrown" bugfix. This will make it easier to revert back to using EarthManipulator? if the "thrown" bug is fixed. Note that CFvw3DManipulator could not be subclassed from EarthManipulator? due to the fact that its data members are marked private and no accessors exist for them.

This commit makes GDAL 1.9.1 RC2 build in our source tree. Look at the changes in this revision to see exactly how I had to modify the GDAL files to build. Note that I copied the GDAL source tree into our repository verbatim and only had to make a few changes to make it build, all of which may be seen by looking at these changes. This change set should reflect the minimum amount of changes necessary for GDAL to build in our tree.
